Our agency is expanding, and we are looking for an experienced Occupational Therapist to provide home health services focused on improving patient independence and safety within their home environment. Responsibilities:&nbsp;
<ul>
<li>
Conduct in-home evaluations and functional assessments&nbsp;
</li>
</ul>
<ul>
<li>
Develop and implement individualized treatment plans&nbsp;
</li>
</ul>
<ul>
<li>
Assist patients with ADLs (bathing, dressing, grooming, toileting)&nbsp;
</li>
</ul>
<ul>
<li>
Train patients on adaptive equipment and home safety modifications&nbsp;
</li>
</ul>
<ul>
<li>
Complete OASIS documentation as required&nbsp;
</li>
</ul>
<ul>
<li>
Collaborate with nurses, PTs, and other team members&nbsp;
</li>
</ul>
 Qualifications:&nbsp;
<ul>
<li>
Graduate of an accredited Occupational Therapy program&nbsp;
</li>
</ul>
<ul>
<li>
Active Florida OT license&nbsp;
</li>
</ul>
<ul>
<li>
Home health experience preferred&nbsp;
</li>
</ul>
<ul>
<li>
OASIS experience strongly preferred&nbsp;
</li>
</ul>
<ul>
<li>
Strong documentation and time management skills&nbsp;
</li>
</ul>
<ul>
<li>
Valid driver’s license and reliable transportation&nbsp;
</li>
</ul>
