We are seeking a dedicated and empathetic Hospice Medical Social Worker (MSW) to join our team. This role plays a vital part in supporting patients and families facing terminal illness. The MSW will assess psychosocial needs, provide emotional support, assist with end-of-life planning, and coordinate community resources.
Key Responsibilities:
- Conduct psychosocial assessments for hospice patients and families.
- Develop and implement individualized care plans in coordination with the interdisciplinary team.
- Provide counseling and emotional support related to grief, loss, and coping.
- Assist families with advance directives, funeral planning, and access to financial, legal, or community services.
- Participate in team meetings and contribute to holistic care planning.
- Maintain accurate and timely documentation in accordance with regulatory requirements.
Qualifications:
- Master’s Degree in Social Work (MSW) from an accredited institution.
