We're looking for passionate Home Health Care Managers who want to make a real difference in the lives of children and families. In this role, you'll serve as a guiding force for families navigating complex systems, helping them access vital services and supports that improve their health and overall well-being. You'll coordinate and oversee services for your assigned caseload, build trusted relationships with both families and service providers, and connect individuals to essential resources such as housing, financial support, and substance use services. Additionally, you'll track outcomes and ensure that all interventions are both effective and compassionate, making a lasting impact through every interaction.

Education and Experience:

• Bachelor Degree in Human Service or related field with two years of experience or
• Registered Nurse or Licensed Practical Nurse with one year of experience.

Additional Requirements:

• Relevant expertise and experience in serving children and families in child welfare, developmental disabilities, mental health, healthcare and/or other systems as well as those receiving preventive services.
• Experience providing service coordination and information, linkages, and referrals for community-based services.
