Sunbelt Staffing is seeking a skilled and compassionate Home Health Registered Nurse (RN) for a 13-week, full-time contract assignmentinHartford, KY.Join a respected national home health agency and provide personalized care to patients in the comfort of their homes. Contract Details:
- Location:Hartford, KY, and surrounding areas
- Company: Sunbelt Staffing, in partnership with a nationwide home health agency
- Contract Length: 13 Weeks (with possible extension upon successful start and completion)
- Hours: Full-time, 40 hours per week, Monday-Friday, 8 am-5 pm, with occasional weekends and on-call duty (typically one weekend per month on-call)
- Productivity: 25 per week
- Compensation: Negotiable, based on experience and territory budget
- Mileage Reimbursement: 0.48c per mile, 150-250 miles per week expected
- Blended Pay Packages: Available for travelers
- Benefits: Medical, dental, vision, 401 (k) match after 90 days
Job Responsibilities
- Conduct patient assessments using OASIS, ensuring accurate and timely documentation.
- Develop and implement personalized care plans in collaboration with physicians and other healthcare professionals.
- Provide skilled nursing care, including wound care, medication administration, and monitoring of vital signs.
- Educate patients and their families on care plans, medication management, and other health-related topics.
- Maintain accurate patient records in Homecare Homebase (HCHB) and ensure compliance with agency standards.
- Travel to patient homes within thearea, utilizing reliable transportation.
- Participate in case conferences and team meetings to ensure coordinated and comprehensive care.
Requirements
- Active Registered Nurse (RN) license in the state of Kentucky.
- Minimum of 1 year of home health experience working with adult and geriatric patients.
- Proficiency in OASIS and Homecare Homebase (HCHB) required.
- Oasis Certified strongly preferred.
- Current American Heart Association or American Red Cross Basic Life Support (BLS) certification.
- Reliable transportation with a valid driver's license.
- Ability to meet Joint Commission onboarding standards upon offer.
