JOB TITLE: RN Case Manager

  • Requires occasional Skilled Nursing visits in most areas.

JOB LOCATION OPTIONS:

  • Akron/Canton + up to 1-hour travel radius
  • Cleveland + up to 1-hour travel radius
  • Norwalk with 1-2-hour travel radius (negotiable)

SCHEDULE TYPE: Customizable. We'll build a schedule -close to home- that fits your availability, whether that's on weekdays or weekends, shifts during the day, evenings, or nights - or a hybrid.

EMPLOYMENT TYPE: Long-term employment for Full-time or Part-time positions

  • No On-Call requirements / No Overnight shifts required.
  • Weekend services are optional when available but are not required.
  • Overtime, Respite & PRN services are offered when available but are not required.

Home Care Network (HCN) is seeking a highly skilled, experienced RN Case Manager to join our home health agency and serve existing and new clients across NorthEast / North Ohio. This is a long-term employment opportunity that can be built to fit your schedule (Full time or Part time) - whether that's:

  • as a consistent primary/sole job
  • as a 2nd job (working around your primary schedule)
  • starting at Part-Time and increasing to Full-Time in the first few months
  • or starting and staying at Full-Time

More about the role:

  • As a Part-Time position - pay is hourly, every week. Typical travel distances include a 30-60 minute radius daily
  • As a Full-Time position - pay can be hourly or salary, both also paid every week. Typical travel distances include a 60-90 minute radius daily + up to 2 hours on occasion.
  • Services include starts of care (SOC), recertifications, resumptions of care (ROC), and other assessments and services, as well as new hire orientations and reviews. *In many locations, services may also require a few weekly Skilled Nursing visits.
  • Typical hours are between Monday - Friday (weekdays) during the day (8am - 5pm).
  • Round trip / Door-to-Door mileage reimbursement provided.
  • At least 1 year of experience in case management is required.
    • 1 or more years of experience in home health care, wound care, lab draws, and OASIS assessments is a plus.

Compensation:

  • Part-time & Full-time Hourly: starting at $35/hr
  • Full-time Annual Salary (+ PTO): starting at $72,800 (*requires pre-determined minimum visits/services per week)

Job Summary
As an RN Case Manager, you will be instrumental in coordinating and providing patient care and ensuring optimal outcomes.

  • With core skills in managed care, patient assessment, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.
  • Your premium skills in critical care and infectious disease care will be valuable in handling complex cases, while your relevant skills in home and community care and supervising experience will enhance your ability to lead and support a multidisciplinary team.
  • You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.

Reports to: Regional Administrator, or Clinical Administrator/Director of Nursing


Responsibilities/Requirements of an RN Case Manager / Clinical Supervisor include:

  • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • RN to Document/Communicate/Coordinate patient status/change in condition to care team.
  • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
  • Minimum of one (1) years of nursing supervisory experience. Home care experience preferred.

Qualifications

  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • 1 year Experience in case management required, with home health experience highly preferred.
  • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability track record with previous employers.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current Driver's License and reliable transportation.
