Job Summary: This Clinical Nurse Supervisor is responsible for providing individualized care for each resident at Bethesda North Pointe in an assisted living and memory care setting. This role is responsible for monitoring all health changes of residents; communicating changes to resident’s provider, family, and other appropriate personnel; appropriately and safely perform procedures and treatments as ordered by physician; ensuring documentation is recorded appropriately and timely; and supervising and providing support to licensed nurses and unlicensed caregivers.

Job Relationships:

1. Reports to: Director of Bethesda North Pointe
2. Supervises: Licensed nurses and unlicensed caregivers
3. Internal Relationships: Receptionist/Scheduling Coordinator, Social Service Worker Housing Division licensed nurses, other department supervisors
4. External Contacts: Resident family members, physicians, and other service providers.

Responsibility: Monitor, direct and manage the care of residents.

Tasks:

1. Complete admissions and ongoing nursing assessments for residents.
2. Review health status of residents regularly and make appropriate referrals to ensure maximum health and wellness along with implementing other services as needed.
3. Monitor residents to assess their medication and care plan.
4. Communicate with families regarding changes in health or care plan.
5. Communicate and consult with physicians, contracted therapy, and other service providers to best meet resident needs.
6. Seek appropriate interventions as needed.
7. Actively participate in quality improvement initiatives.
8. Promotes and implements policy & procedures under the direction of the Director of Bethesda North Pointe that assist the residents in achieving their highest practicable physical, psychosocial, and spiritual function.
9. Establish an individualized care package for nursing services that meets resident needs.
