Position Overview
As an Occupational Therapist, you will leverage your clinical skills to assess, teach, and deliver therapy interventions as directed by the physician. Collaborating with an interdisciplinary team, you will share updates and insights to optimize patient well-being.
Schedule: Full Time, Monday - Friday
Territory: Arcadia and Punta Gorda, FL
Essential Job Functions:
- Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels.
- Administer, evaluate, and interpret tests to determine functional goals and outcomes.
- Develop a comprehensive written evaluation and plan of care outlining occupational therapy needs, goals, progress, and expected outcomes.
- Deliver occupational therapy services in compliance with prescribed physician orders.
- Maintain the plan of care through timely documentation, modifying goals and treatment as necessary.
- Oversee discharge planning, educating patients and caregivers on home exercise plans.
- Coordinate services with healthcare teams, including physicians, nurses, and family members.
- Document according to policy, ensuring minimal supervision and no technical or clinical denials.
- Foster effective communication among staff, healthcare team members, and families.
- Supervise other personnel (OTA, HHA) as applicable.
- Complete OASIS assessments where permitted by state regulations.
Requirements:
- Degree in Occupational Therapy from an accredited university/program.
- Current, unrestricted state license as an Occupational Therapist in the state of practice.
- Valid CPR certification.
Preferred Qualifications:
- Strong organizational and communication skills.
- Attention to detail and effective time management.
- Proven ability to build relationships.
- Experience in Home Health Therapy and completion of OASIS documentation.
