This request is to fill an upcoming short-term leave and is anticipated to only be for 6 months.
• Maintain a caseload of 6 children in the IDT program within the elementary classroom, kindergarten - 6th grade
• Participate in IDT admissions with child, family, MD, and school liaison
• Provide Weekly Individual Therapy
• Provide Daily Group Therapy
• Provide Family Therapy and Crisis Intervention as needed
• Facilitate Transition Meetings for each student on their caseload that include the student and family, IDT teacher, school liaison and other professionals from the student’s home district
• Collateral involvement with participating schools and agencies
• Function as part of an interdisciplinary treatment team
• Participate in weekly Treatment Team meetings
• Advocate for youth and families to obtain community-based linkages with additional supportive services that meet their needs
• Maintain timely medical record documentation
• Facilitate thorough discharge planning and complete discharge follow-up
• Collaborate with school liaisons throughout Dutchess County to facilitate the referral and screening process.
