We’re seeking a bold, compassionate Family Nurse Practitioner (FNP) to join a fast-paced, patient-first Direct Primary Care (DPC) team in Metro Detroit. This role is ideal for a provider who values relationship-based medicine and is committed to delivering high-quality care to a diverse community.
Responsibilities

Provide comprehensive primary care to patients across the lifespan

Deliver culturally responsive, patient-centered care

Assess, diagnose, treat, and prescribe medications as appropriate

Educate patients and families on prevention, wellness, and chronic disease management

Document care accurately and efficiently using established workflows

Collaborate with clinical and administrative teams to support continuity of care

Support patient communication in English and Spanish
RequirementsQualifications
Required
- Active Michigan Family Nurse Practitioner (FNP) license
- Master’s Degree in Nursing or related field
- Bilingual in English and Spanish
- Strong clinical judgment, patient advocacy mindset, and commitment to relationship-based care
Preferred
- 2–5 years of experience in Primary Care
- Experience in outpatient, community-based, or value-based care models
- Comfort working in a fast-paced but patient-focused clinical environment
