\n\nJob Requirements (RN)\n

- Experience: Minimum two (2) years bedside clinical experience (highly desired)\n
- License: Active RN license\n
- Certifications: BLS (required prior to first shift); ACLS required prior to first shift (CIRH only)\n
- Strong clinical assessment, care planning, and nursing documentation skills\n
- Comfort caring for medically complex, high-acuity patients in a long-term acute care environment\n
\n\nResponsibilities\n
- \n
- Complete comprehensive nursing assessments within required timeframes and develop individualized plans of care\n
- Deliver direct patient care using the nursing process (assess, plan, implement, evaluate), including patient and family education\n
- Document assessments, interventions, medications, and patient responses accurately and on time in the medical record\n
- Communicate changes in patient condition promptly to physicians, charge nurse, and the care team to support continuity and patient safety\n
- Participate in interdisciplinary rounds/conferences and provide clear updates for initial and weekly re-evaluation meetings\n
- Initiate discharge planning on admission and reinforce teaching to support safe transitions of care\n
- Delegate appropriately; supervise and support ancillary staff; assist with training and unit teamwork\n
- Promote a safe environment through infection prevention, safe patient handling, and hazard awareness\n
- May assume charge nurse responsibilities after orientation, as needed\n
