Physical Therapist – Home Health

Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives.
- Serve as co-leader in an interdisciplinary approach to patient groups, coordinating care with other clinicians.
- Conduct comprehensive home-based PT evaluations, develop individualized treatment plans, and implement therapies to increase functional mobility and activities of daily living.
- Educate patients and families on prescribed exercises, safe mobility, fall prevention, and home safety to support ongoing progress.
- Document patient progress in medical records, ensure accuracy of treatment notes, and maintain compliance with regulatory and agency requirements.
- Maintain a safe and effective therapy environment within the home setting and advocate for patient well-being throughout the course of care.
Requirements
- Active Physical Therapist license in California
- Ability to work in a home health/hospice setting
- Availability to provide 40 hours per week in a travel assignment (Days, 8 hours)
- Willingness to travel to patient homes within the service area as part of a travel assignment
- Ability to collaborate effectively within an interdisciplinary care team
