Our organization is in search of a committed and empathetic Physical Therapist to become a part of our Healthcare & Medical Services team. This role focuses on delivering exceptional, patient-centered rehabilitation services across various care environments.
Key Responsibilities
- Conduct thorough evaluations to assess patient impairments, functional limitations, and personal goals.
- Develop and modify individualized treatment plans based on clinical expertise and patient needs.
- Implement evidence-based physical therapy interventions aimed at enhancing recovery, functional independence, and overall well-being.
- Provide hands-on treatment, including therapeutic exercises, manual therapy, gait and balance training, and functional mobility training.
- Monitor patient progress closely and adjust interventions as necessary.
- Educate patients, families, and caregivers about conditions, home exercise programs, and strategies to improve safety, mobility, and quality of life.
- Collaborate with a multidisciplinary team, including occupational and speech therapists, nurses, and physicians, to create coordinated care plans.
- Participate in case conferences and interdisciplinary meetings to enhance patient outcomes.
- Maintain accurate and timely documentation in the electronic medical record to ensure continuity and quality of care.
Requirements and Qualifications
