PACE North is seeking a compassionate and dedicated Nurse Practitioner or Physician Assistant who is motivated to make a meaningful impact in the lives of older adults in our community.

What is PACE North?

PACE stands for Program of All-Inclusive Care for the Elderly. We are:

  • A unique care model for individuals aged 55+ who qualify for nursing facility-level care but prefer to live at home
  • A growing organization, launched in 2019
  • Focused on providing personalized care in a variety of settings: in our clinic, in the home, or in skilled nursing facilities

As a Primary Care Provider at PACE North, you will:

  • Work as part of a multidisciplinary team and collaborate with other professionals to meet the emotional, physical, social, and medical needs of participants
  • Deliver care in multiple settings:
    • On-site clinic
    • Participant homes
    • Partner skilled nursing facilities
  • Participate in a rotating after-hours on-call schedule to support the on-call nurse
  • Build strong relationships with participants and their families through education and support
  • Practice in a flexible care model that adapts to participant needs rather than rigid systems

Requirements:

We’re looking for a compassionate and collaborative healthcare professional with the following:

  • A passion for serving others with a positive, person-centered approach
  • Commitment to interdisciplinary decision-making that prioritizes:
    • Participant goals
    • Respect for self-determination
  • Graduate of an accredited Nurse Practitioner or Physician Assistant program
  • Current and valid Michigan license as a Nurse Practitioner or Physician Assistant
  • Strong clinical leadership skills
  • Excellent interpersonal and communication skills
  • Effective problem-solving abilities
  • A collaborative mindset with a focus on shared decision-making
  • Experience in primary care or geriatric care (preferred)
  • An inspirational, motivational, and creative approach aligned with the PACE model of care
