Wage range $25.85 to $29.89 depending upon verifiable experience.
We are also offering a $6,000 sign on bonus that is paid over the course of one year.

Key Responsibilities
• Provide direct nursing care, including assessments, clinical procedures, monitoring, and individualized care planning.
• Administer medications safely and accurately, follow drug control protocols, and document all treatments and resident responses.
• Observe, document, and communicate changes in resident condition to the appropriate clinical staff and families.
• Supervise, support, and delegate tasks to TMAs and Nursing Assistants; ensure quality and compliance with care plans.
• Respond to medical emergencies, implement appropriate interventions, and coordinate care.
• Maintain complete and accurate clinical records, vital signs, charting, and physician orders.
• Promote resident rights, privacy, dignity, safety, and confidentiality, adhering to HIPAA and all regulatory standards.
• Participate in quality improvement efforts, staff meetings, and required in-service training.
• Follow infection control, safety, and emergency procedures; report incidents, hazards, or equipment issues promptly.
Qualifications
• Active, unencumbered LPN license in Minnesota.
• Current CPR certification.
• Strong communication, assessment, and documentation skills.
• Ability to work independently, supervise staff, and manage multiple priorities.
• Compassion, professionalism, and the ability to work effectively with older adults and individuals with diverse needs.
• Physical ability to lift/move 30–60 lbs (with assistance), walk/stand for extended periods, and perform essential job functions safely.
