Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans.
- Provide skilled nursing care according to established protocols and physician orders.
- Coordinate with interdisciplinary teams and communicate effectively with patients and their families.
- Maintain accurate and timely documentation of patient care and progress.
- Ensure compliance with safety, infection control, and regulatory standards.
- Manage equipment and supplies necessary for home health visits.
Requirements
- Current Michigan Registered Nurse License.
- Valid Driver’s License and reliable transportation.
- Strong clinical assessment and critical thinking skills.
- Good communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Must pass background check and drug screening.
