Position: Case Manager/Group Facilitator

Position Classification: Exempt
Position Overview:
The Case Manager/Group Facilitator works closely with the clinical team to provide a cohesive treatment experience for clients. This role combines Case Management and Group Facilitation responsibilities, including intake assessments, treatment planning, coordination of care, individual and group counseling, and aftercare planning. The Case Manager/Group Facilitator ensures patients receive high-quality, person-centered services that support recovery and wellness in line with the mission and values of New England Medical Group.

Specific Responsibilities:
- Act as the patient’s advocate regarding insurance coverage, benefits, and available resources.
- Complete intake documentation and assessments.
- Provide individual and group counseling, skills training, and crisis intervention.
- Collaboratively develop, implement, and update individualized treatment plans with patients.
- Coordinate and facilitate patient care through assessment, planning, and implementation.
- Communicate patient needs to team members and follow up accordingly.
- Maintain accurate and timely documentation in the Electronic Medical Record (EMR).
- Collaborate with Community Relations team for referrals and aftercare planning.
- Provide discharge planning and coordinate community reintegration services.
- Update referring sources and family members on patient progress (with consent).
- Use motivational interviewing and other evidence-based interventions to support recovery.
- Follow up on missed groups, outreach to clients, and document in EMR.
- Send client satisfaction surveys and gather feedback at discharge.
- Participate in team meetings and provide updates on admissions, discharges, and client leads.
- Gathering and completing paperwork required for FMLA, PFML, STD or other leave-related tasks.
Education and Experience:
• Bachelor’s Degree in Social Work, Mental Health, Addictions, Psychology, or related field preferred.
• LADC I or II preferred (not required).
• 1–2+ years of experience in behavioral healthcare or substance use disorder treatment settings.
• Knowledge of PHP/IOP/OP best practices and case management for dual-diagnosis clients.
• Proficiency in Electronic Medical Record (KIPU or equivalent).
• Strong organizational, communication, and problem-solving skills.
• Ability to work effectively in a fast-paced, multidisciplinary team environment.
• Understanding of diverse cultures and gender-specific needs.
• Commitment to confidentiality and ethical standards.
Work Environment:
This position operates in a professional office and clinical environment. Standard office equipment such as computers, phones, photocopiers, and filing cabinets are routinely used.
Physical Demands:
While performing the duties of this position, the employee is regularly required to talk or hear. The employee frequently is required to use hands to handle objects, tools, or controls. The employee is occasionally required to stand, walk; sit; reach with hands and arms; climb or balance; stoop, kneel, crouch, or crawl. The employee must occasionally lift and/or move objects up to 25 pounds. Specific vision abilities include close vision, distance vision, color vision, peripheral vision, and the ability to adjust focus.
