Job Description:
Sirius Home Care is seeking a dedicated and experienced Qualified Professional (Nurse) to join our team under the Community First Services and Supports (CFSS) program. In this role, you will play a crucial part in ensuring quality care for individuals receiving services by creating care plans, supervising workers, and maintaining high standards of care.
Key Responsibilities:
- Assist individuals in adding details to their Person-Centered Plan, ensuring it meets their unique goals and needs.
- Oversee the delivery of care to ensure services align with the care plan and quality standards.
- Supervise workers, providing guidance and support to ensure services are delivered effectively and safely.
- Assess and ensure worker competence, offering training and feedback as needed to maintain high standards of care.
Qualifications:
- Licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Minnesota.
- Experience in home care, case management, or a similar field is preferred.
- Strong leadership, organizational, and communication skills.
- Ability to work independently while managing multiple responsibilities.
- Knowledge of CFSS or similar programs is a plus.
- Must pass a background check and meet all state requirements for the Qualified Professional role.
