Licensed Practical Nurse (LPN) – Per Diem – Goshen, NY

Star Nursing is seeking LPNs for urgent per diem opportunities at a Sub-Acute Rehab Facility in Goshen, NY (10924). Full-time block booking is available, offering the opportunity to secure a steady schedule and potentially exceed 40 hours per week.

Job Details:

• Location: Goshen, NY

• Facility: Sub-Acute Rehab

• Position: Licensed Practical Nurse (LPN)

• Job Type: Per Diem

• Shifts Available:

  • Overnight (11 PM – 7 AM)

  • PM Shift (3 PM – 11 PM)

  • AM & PM Weekend Shifts

Pay Rates:

• Per Diem Rate: $45 per hour (taxable)

• Split Pay Rate: $16/hr. (taxed) + $1,240 per week (tax-free) $16/$31 = $47/hr.

Job Responsibilities:

• Provide direct nursing care under RN or physician supervision

• Monitor and document vital signs

• Observe and report changes in patient conditions

• Assist with wound care, dressing changes, and other medical treatments

• Administer prescribed medications and monitor for adverse reactions

• Assist patients with mobility and personal hygiene tasks

• Maintain accurate and up-to-date patient records

• Ensure compliance with facility policies and infection control protocols

Requirements:

Step 1:

• Resume with long-term care experience

• Active LPN license

• Current BLS/CPR certification

• Skills checklist

• 1-2 recent supervisor references

Step 2 (Upon Offer):

• Physical and PPD (within the last 12 months)

• MMR, Varicella, Hep B (or decline)

• Tdap/Flu (or decline)

• COVID negative test (current)

• LGBT Training Certification

• Background check and urine drug screen

• Copy of driver’s license and Social Security card
