Job Description
The Therapeutic Case Manager partners with youth to assess and identify their needs and create shortterm goals. They consult with youth as appropriate, to arrange, coordinate, monitor, evaluate, and advocate for services to meet their specific needs.

Program Delivery
- Manage a caseload of up to 10 youth and provide trauma-informed counseling on a weekly basis for each youth.
- Work closely with each youth’s parent/guardian and/or case worker to coordinate care while the youth is in the shelter.
- Schedule child and family team meetings, regularly updating the guardian or Social Worker on the youth’s progress and work to develop a transition plan.
- Utilize a “wrap-around” service model to develop and coordinate family meetings towards the achievement of client goals.
- Lead psycho-educational groups as requested.
- Participate in weekly case staffing meetings with the Youth Haven employee team through the provision of progress reports, discharge plans and recommendations on how to effectively work with youth in meeting their goals.
- Meet weekly with the Manager to provide client updates and discuss best practices for providing case management and therapeutic services with the shelter.
- Assist in floor coverage and supervision of youth as scheduled or needed.
- Participate in on-call rotation to assist in the referral process and intakes as needed.
Case Management Tasks
- Provide written assessments, treatment plans, goal plans and discharge reports per program and contract requirements.
- Make referral for services of unmet needs identified through assessments and counseling sessions.
- Collaborate with youth and their service team to develop goals and create an appropriate transition and discharge plan.
- Link youth and their service team with community resources as needed.
- Provide referrals for aftercare services for youth, their family, and their service team.
- Ensure reports and progress notes are completed and submitted in a timely manner.
- Maintain case files in an organized and confidential manner.
- Provide support and training to employees as requested.
- Assist in floor coverage and supervision of youth as scheduled or needed.
- Attend provider meetings in the community and assist in outreach efforts to help ensure awareness of youth shelter services.

job requirementsa. Education and Experience
- Bachelor’s degree in a human-service field preferred.
- Minimum of 2 years of relevant working experience, including supporting youth, young adults and families experiencing homelessness.
- Experience providing case management services with an emphasis on trauma-informed, positive youth development, housing first and harm reduction.
- Demonstrated knowledge of MS365 Power Platform.
b. Additional Requirements
- Must satisfactorily pass criminal history check.
- Must satisfactorily pass tuberculosis (TB) test.
- Must have reliable transportation and a valid Washington State driver's license (or be able to obtain one by date of employment).
- Employees and volunteers who operate their own or Friends of Youth vehicles on Friends of Youth business, must have a safe driving record as defined by Friends of Youth.
- Employees and volunteers who operate their own vehicle(s) on Friends of Youth business must carry auto liability insurance that complies with the requirements of Friends of Youth.
- Must successfully complete the following health certification requirements within 30 days of employment: CPR, First Aid and Bloodborne Pathogens.
