Job Summary: Under the general supervision of the Director of Nursing, the Nurse Supervisor assists with planning, organizing, directing, coordinating, and evaluating nursing services and educational programs within the transitional care division of Care Management. The role involves working with the transitional care team to reduce 30-day readmissions and serve high-risk patients post-discharge from inpatient med/surg units and the Adult Emergency Department. The Nurse Supervisor organizes and oversees care coordination to optimize patients' health in the community.
Required Qualifications:
- Seven (7) years of experience, including five (5) years in a supervisory role.
- Two (2) years of experience in discharge planning and utilization review.
- CCM Certification.
- PRI Certification.
- Bachelor's degree in Nursing from an accredited Nursing Program.
- RN License.
- Primary Source Verification of License and Registration of Registered Professional Nurse from NYS Department of Education (NYSED).
- BLS Certification.
Preferred Qualifications:
- Master's degree in Nursing.
Additional Information:
- Provides oversight and direction to Accountable Care Managers (RNs), Care Coordinators (CLWs), clerical staff, and ancillary staff to meet transitional care division goals.
- Participates in developing policies and standards for nursing service and education in care management and transitional care.
- Performs all duties of the Director of Nursing in their absence.
