Responsibilities

  • Provide patient care under supervision, ensuring safety and comfort.

  • Monitor vital signs and document changes in patient condition promptly.

  • Support care plans by administering prescribed treatments and medications.

  • Adhere to regulatory and safety standards; ensure infection control compliance.

  • Maintain accurate, timely documentation in patient records and logs.

  • Collaborate with healthcare team to identify escalating patient needs.

  • Demonstrate accountability for personal performance within a team-driven environment.

  • Participate in performance metrics and quality improvement activities.

  • Educate patients and families on care procedures and post-discharge steps.

  • Utilize care pipelines and territory insights to anticipate patient needs.

  • Support the development of care plans aligned with growth and merit-based advancement.


Experience/Qualifications

  • Licensed Practical Nurse (LPN) with active state license.

  • Adhere to infection control standards and regulatory requirements.

  • Provide patient care under supervision with emphasis on safety and comfort.

  • Monitor and document vital signs and patient condition changes promptly.

  • Administer prescribed treatments and medications per protocol.

  • Accurate and timely documentation in electronic health records.
