Job DescriptionLicensed Practical Nurse (LPN) – Behavioral Health

Shifts: 12-Hour Day & Night Shifts
Location: Hopkinsville, KY

Position Summary

The LPN plays a key role in supporting patients’ mental and physical well-being by providing hands-on care, monitoring conditions, and assisting the healthcare team in delivering effective treatment.

Responsibilities:

• Provide direct patient care and monitor behavioral and physical health status

• Administer prescribed medications and monitor patient responses or side effects

• Accurately document care provided and report changes in patient condition to the RN or physician

• Review and transcribe physician orders

• Assist with patient assessments and care planning

• Work collaboratively with the interdisciplinary healthcare team

• Educate patients and families about treatment and medication plans

Qualifications:

• Active, unencumbered LPN license (required)

• Graduate of an accredited nursing program

• Strong communication and clinical documentation skills

• Ability to maintain professionalism and clear boundaries

• Experience in behavioral health or psychiatric care preferred
