PHA Health, in partnership with Wellness Partners LLC, is looking for an experienced and dedicated Nurse Practitioner (NP) with a passion for prevention and patient access to affordable care to join our team. You must live in Nebraska or a surrounding state to be considered for this position. In this role, you will utilize telephonic coaching skills to help participants understand their current health status by developing a health coaching relationship and assisting the participant through the process of actively working toward improved health through support, encouragement, and education. The Nurse Practitioner will also work as part of the health fair team to manage and develop a supportive and pleasurable health screening environment for participants as well as our own staff.
Responsibilities:
- Facilitate new patient consultation and health history review
- Order and administer tests including bloodwork
- Review and interpret test results to diagnose health problems
- Develop relationships with and motivate participants to recommend further treatment options or future appointments if needed
- Evaluate and interpret participant lab data (trend past data as available), educate participants on the results, and encourage follow-up with a healthcare provider as appropriate.
- Provide after-care/ongoing care information and resources to patients and their families, including Telephonic coaching, or referrals to other practitioners when necessary
- Perform follow-up calls as appropriate to assess health status and/or goal progress.
- Ensure patient records are kept up to date including test results, immunizations, condition changes, etc.
- Distribute educational materials to participants as needed.
- Oversee clinic team, including phlebotomists.
- Administer vaccines
Additional Responsibilities:
- Occasional travel as part of a team manage and conduct preventive care clinics.
- Prioritize multiple time-sensitive projects at once.
- Contribute to quality control efforts.
- Must have some evening availability.
Minimum Requirements:
- Proficient in Microsoft Word & Excel.
- Strong communication skills, both verbal and written.
- Occasional travel and public speaking.
- Must carry Professional Liability Insurance.
- Able to perform medium work (exerting up to 35 pounds of force).
- Able to sit and stand for long periods of time.
- Have adequate knowledge of organizational policies, regulations, and procedures.
- Establish and maintain effective working relationships with participants, medical and clinic staff and the public.
Qualifications:
- Master's degree in nursing
- Current state nurse practitioner license
- 5+ years of nursing experience
- Understands HIPAA and OSHA regulations
- Able to operate as part of a team, alongside physicians and other nurses
- Proficient in computer-based data management programs and information systems
