Job Summary:
This position coordinates, develops, and oversees re-entry services provided by the care management team to client partners of Second Chance Center (SCC) services. The Director of Care Management at the MOORE Center will establish and maintain collaborative partnerships with community organizations to assist in the transition of client partners towards independence and stability. The Director of Care Management will ensure SCC care management services at the MOORE Center adhere to Community Corrections standards, while maintaining SCC’s person-centered, evidence-based practices. The Director of Care Management at the MOORE Center is an SCC leadership team member who will participate in fiscal management, quality assurance, and reporting.

Supervisory Responsibilities:
- Direct the activities of the care management team and supervise staff performing daily case management functions; provide clear objectives and support as needed.
- Monitor the performance of all supervised staff; conduct performance evaluations and develop individualized professional development plans.
- Monitor workloads to ensure performance requirements are met and workloads are balanced.
- Serve as a liaison between Care Management and other SCC departments, developing collaborative approaches to provide a holistic approach to meet client needs.
- Provide necessary support to all supervised staff in various circumstances/ settings, including emergency and crisis situations.
- Responsible for the fiscal management of the departmental budget in compliance with applicable grants and contracts.
Duties/Responsibilities:
- Communicate and collaborate with City of Denver Operations staff to implement re-entry services in a Community Corrections setting.
- Develop and implement policies and procedures to ensure that programmatic and contractual goals are achieved.
- Ensures data integrity and compliance with community correction standards as well as internal policies and procedures.
- Implement and maintain a quality assurance process in cooperation with the data administration team.
- Ensures an appropriate distribution of workload amongst the care management team.
- Oversee residential programming and collaborates with multiple stakeholders and partners to provide additional services to residents.
- Actively participates in staff recruitment and retention efforts.
- Manage program budgets to ensure fiscal responsibility and sustainability.
- Provide guidance on effective care management strategies to ensure participant engagement.
- Participate in grant writing and reporting for programmatic expansion and development.
Required Skills/Abilities:
- Excellent time management skills with a proven ability to meet deadlines.
- Excellent interpersonal skills and demonstrated ability to effectively build rapport.
- Excellent verbal and written communication skills; able to effectively communicate with community partners and stakeholders, staff, families, and enrolled participants.
- Experience working directly with systemically marginalized individuals.
- Demonstrated history of effective leadership and managerial practices.
- Ability to work easily with a wide variety of people.
- Ability to establish priorities and delegate tasks in an environment of competing demands.
- Previous experience working with the re-entry population in a lead capacity.
- Previous experience in a supervisory capacity with program evaluation and service coordination duties.
- Understanding of trauma-informed and strengths-based care management practices.
- Proficiency with Microsoft Office (Word, Excel, Outlook, etc.) and ability to use these and other required programs at an intermediate to advanced level.
- Commitment to SCC’s mission and the principles of diversity, equity, and inclusion.
Preferred Skills / Competencies are as follows:
- Bilingual (Fluent Spanish) preferred.
- Knowledge of Case Management databases.
- 3-5 years of care management experience.
- Previous experience working with the re-entry population.
Required Education and Experience:
- 5+ years of relevant experience working in adult re-entry or with marginalized individuals.
- Related undergraduate degree or equivalent experience including lived experience.
- 3+ years of supervisory/managerial experience.
Physical Requirements:
- Occasionally required to stand, walk, and lift up to 20 pounds.

Compensation & Merit Increases:
- The pay range for this position is $75,000 - $90,000 per year
- Competitive pay commensurate with education and experience.
- Vacation Leave & Sick Leave, Medical, Dental & Vision Insurance, 401k Plan.
- Merit increases are based on annual performance reviews.
- Cross-training and departmental support are integral parts of this role and may be assessed as part of ongoing performance evaluations.
- Employees brought in at the top of their pay range will need to transition to a role with greater responsibilities to qualify for further raises.
Position Type/Expected Hours of Work
- This full-time position requires a minimum of 40 hours per week.
- Some evening and Saturday work may be required.
- Work hours are 8:00AM – 4:30PM, Monday through Friday, and half days every other Friday and Saturday 8:00AM – 12:00PM, but may be adjusted according to business needs.
- This role is in-office.
