Location: Clive, IA
Job Type: Travel Contract
Duration: 13 weeks
Shift: 5x8s Days
Pay: $2,145/week

Job Description

We are seeking a skilled Physical Therapist to join a skilled nursing and long-term care facility in Clive, IA, on a travel contract. The Physical Therapist will evaluate residents, develop individualized care plans, and provide therapy services to improve mobility, strength, balance, and independence. The ideal candidate is compassionate, adaptable, and experienced in geriatric rehabilitation.

Key Responsibilities

  • Perform initial evaluations to assess residents’ physical abilities, mobility, and therapy needs.
  • Develop and implement customized treatment plans in collaboration with physicians and interdisciplinary staff.
  • Deliver therapy interventions including gait training, strengthening, balance exercises, and functional mobility activities.
  • Educate residents, families, and caregivers on therapy objectives, safe mobility, and assistive device use.
  • Document evaluations, progress notes, and therapy sessions accurately and in compliance with facility and regulatory standards.
  • Collaborate with occupational therapy, speech therapy, and nursing staff to ensure comprehensive, patient-focused care.
  • Promote independence, safety, and improved quality of life through evidence-based therapy practices.

Qualifications

  • Doctorate or Master’s degree in Physical Therapy from an accredited program.
  • Current and valid Physical Therapist license in Iowa (or compact license).
  • Minimum of 1 year of recent PT experience, preferably in LTC/SNF settings.
  • Current certification in Basic Life Support (BLS).
  • COVID-19 vaccine and recent tuberculosis test.
  • Strong clinical assessment, documentation, and communication skills.
  • Compassionate, resident-centered approach with geriatric rehabilitation experience preferred.
