Position Overview
Are you passionate about nursing and making a difference in the lives of people experiencing homelessness? Join Fourth Street Clinic's Medically Vulnerable Populations (MVP) program-a-first-of-its-kind, multi-partner respite and recuperative care facility in Sandy, Utah.
You'll collaborate with Fourth Street Clinic staff, community agencies, local hospitals, and homeless resource centers to deliver high-quality, coordinated health care. At MVP, no two days are the same - you'll work alongside EMTs, case managers, and providers to provide triage, wound care, chronic disease management, medication education, and more.
Training begins at our downtown Salt Lake City clinic, where you'll learn triage, homeless services, and FSC resources before transitioning to the MVP facility.

Core Responsibilities
Patient Care & Clinical Services:
- Conduct patient intake and admission assessments; develop individual respite care plans.
- Provide nursing care, including triage, wound care, chronic disease management, medication education, immunizations, and lab testing.
- Identify and address risk factors, barriers to care, and social determinants of health.
- Follow standing orders, maintain scope of practice, and ensure care aligns with therapeutic standards and the Utah Nurse Practice Act.
Care Coordination:
- Collaborate with community partners to identify and engage patients in need of respite care.
- Ensure continuity by coordinating with FSC teams, external providers, and community agencies.
- Assist patients in navigating appropriate levels of care, services, and facilities (e.g., Home Health, oxygen, PT/OT).
- Access and integrate external health records to support treatment planning.
Treatment Planning & Documentation:
- Develop and regularly update patient-centered treatment plans.
- Document assessments, care plans, interventions, and outcomes in the electronic health record.
- Track and maintain data to support reporting, billing, quality improvement, and value-based care metrics.
Triage & Scheduling Support:
- Provide triage coverage when assigned, including assessing the severity of health concerns, coordinating timely care, and consulting with providers as needed.
- Support front desk staff with appropriate patient scheduling, walk-in triage, and coordination of referral activities.
- Participate in telephone triage and manage medication refill requests in collaboration with providers.
Team & Organizational Responsibilities:
- Participate in staff meetings and other assigned meetings.
- Maintain current license, certifications, and knowledge of FSC policies and clinical systems.
- Promote patient independence by setting care goals and encouraging self-management.
- Contribute to a safe, clean, and compliant work environment.
Leadership:
- Foster a collaborative, trauma-informed culture grounded in respect, transparency, fairness, and learning.
- Mentor and coach EMTs, case managers, and students/externs to strengthen clinical skills, teamwork, and role clarity.
- Serve as a clinical leader at the MVP facility, coordinating across FSC hub services, the Road Home, and community partners.
- Model accountability, professionalism, and effective communication, setting the tone for the care team.
- Act as a liaison between medical providers, housing staff, and patients to ensure medical recovery goals align with social supports.
- Contribute to quality improvement efforts, including outcome tracking, process refinement, and value-based care initiatives.
- Represent FSC and MVP program in community workgroups, research collaborations, and partner meetings.
Qualifications & Competencies
Required:
- Graduation from an accredited program; active RN license in Utah, in good standing with DOPL.
- Strong assessment, organizational, and multitasking skills.
- ACLS/BLS certification (upon hire or within 90 days).
- Familiarity with would care.
- Ability to prioritize using triage principles.
- Adherence to HIPAA and FSC safety standards.
Work Environment
- Primary worksite: MVP facility in Sandy, Utah.
- Secondary worksite: Fourth Street Clinic, Downtown Salt Lake City.
- RN Staff rotating among FSC locations as needed
