We are growing and seeking experienced Home Health Registered Nurses (RN) to join our PRN per-visit team.This position offers flexible scheduling, competitive per-visit pay, and strong clinical support, allowing you to focus on delivering excellent patient care.Pay & Compensation• Routine Skilled Visit: $45+ per visit
• Higher differential rates available for:

• Start of Care (SOC)
• Recertification (OASIS)
• Resumption of Care (ROC)
• Discharge Visits

Additional Pay• Mileage reimbursement per agency policy
• Competitive visit differentials based on complexity and documentation requirements(Rates depend on experience, licensure, and visit type.)RN Responsibilities• Provide skilled nursing visits in patient homes
• Perform patient assessments and monitor clinical changes
• Complete OASIS assessments when assigned
• Perform skilled treatments including:

• Wound care
• Medication management
• Injections
• Catheter and ostomy care
• Disease management and patient education

• Communicate patient status with physicians and clinical leadership
• Educate patients and caregivers on care plans and medications
• Complete accurate and timely documentation in the EMR
• Follow HIPAA, infection control, and safety guidelinesRequirements• Active Texas RN License
• 1+ year home health experience (2+ years preferred)
• OASIS documentation experience required
• Current CPR/BLS certification
• Valid driver’s license and reliable transportation
• Strong clinical assessment and documentation skills
• Ability to work independently in the fieldPreferred Qualifications• Strong wound care experience
• Home health EMR experience
