Registered Nurse (RN) – Home Health
Location: South Dennis, MA
Start Date: ASAP
Position Summary: The Registered Nurse (RN) performs comprehensive nursing assessments upon patient admission, including physiological, psychosocial, environmental, and health history evaluations, to ensure the safety, quality, and effectiveness of care and services. This role integrates assessment data to develop and manage individualized plans of care in alignment with case management objectives and national standards.
Key Responsibilities:

Conduct comprehensive admission assessments covering physical, psychosocial, and environmental factors

Integrate assessment findings to identify patient problems and develop individualized plans of care

Maintain annual HHRG values in alignment with national standards
-
Utilize HHRG values to determine visit frequency and appropriate interdisciplinary involvement
-
Ensure annual patient outcomes meet national benchmarks
-
Refer patients to appropriate community resources as needed
-
Evaluate clinical needs and initiate, implement, revise, and document plans of care accordingly
-
Participate in interdisciplinary case conferences and evaluate plan of care effectiveness
-
Instruct and provide documented supervision of Home Health Aide plans of care every 14 days
-
Notify physicians and other care providers of significant changes in patient condition
-
Complete admissions and required documentation in a timely manner
-
Record clear, concise narrative documentation reflecting skilled care, teaching, and patient/caregiver response
-
Obtain and clarify valid physician orders for all care delivered
-
Plan and coordinate patient discharges; notify patient and physician and document discharge summaries
-
Complete all required discharge paperwork

Perform additional duties as assigned
Qualifications:
-
Successful completion of pre-hire medication competency test
-
Current, active Massachusetts RN license and appropriate specialty credentials
-
Minimum of one (1) year of nursing experience required, or six (6) months with a BSN
-
CPR certification required
-
Home health and IV experience preferred
-
Strong computer skills preferred
-
BSN preferred
