Location: Wyoming County, WV Organization: Southern Highlands Community Mental Health Center (SHCMHC)
Southern Highlands Community Mental Health Center is seeking a compassionate Case Manager/Care Coordinator II – School-Based to support youth and families within a school setting. This role focuses on coordinating care, providing support, and helping students succeed both emotionally and academically.
???? Key Responsibilities
<ul>
<li>Coordinate behavioral health, medical, and social services</li>
<li>Assist with treatment planning under a licensed clinician</li>
<li>Provide individual and group social-emotional learning (SEL) sessions</li>
<li>Support students and families in crisis situations</li>
<li>Collaborate with school staff, families, and providers</li>
<li>Monitor progress and maintain documentation in the EHR</li>
</ul>
???? Qualifications
<ul>
<li>Bachelor’s or Master’s degree in a related field (Social Work, Counseling, Psychology, etc.)</li>
<li>Valid driver’s license</li>
<li>Strong communication and organizational skills</li>
</ul>
⭐ Preferred
<ul>
<li>School-based or youth behavioral health experience</li>
<li>Familiarity with CCBHC</li>
<li>Licensure (LSW, RN, or similar)</li>
