What You’ll Do:
- Conduct thorough home health evaluations using OASIS and develop personalized care plans
- Deliver skilled physical therapy treatments in patients’ homes
- Accurately document all visits, progress, and outcomes
- Collaborate closely with nurses, occupational therapists, and other care team members
- Educate patients and families on exercises, safety, and recovery strategies
Qualifications:
- Active Pennsylvania PT license (or eligibility to obtain)
- Minimum of one year of recent home health experience
- Proficiency with OASIS documentation
- Strong clinical reasoning, communication, and organizational skills
- Dedication to patient-centered care and professional excellence
