Responsibilities:
- Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
- Designs and implements individualized plans of care based on patient assessments and goals.
- Reviews and updates care plans to ensure optimal outcomes.
- Documents evaluations, treatment goals, plans, and updates documentation as needed.
- Directs and mentors support staff to achieve patient goals.
- Adheres to Elara Carings philosophy, mission, and policies to ensure quality of care.
- Provides leadership to staff and promotes physical therapy in the community.
- Performs job duties in physically demanding environments and may work irregular hours, including on-call if required.
Qualifications:
- Graduate of an American Physical Therapy Association (APTA)-approved physical therapy program.
- Current, unrestricted PT license valid in the state of practice.
- Minimum of one (1) year of experience in a healthcare facility.
- Ability and willingness to travel within the branch/office coverage area.
- Must possess a dependable vehicle, valid drivers license, and current auto insurance per state requirements.
- Able to lift, push, pull, and support up to 50 pounds, including assisting in patient transfers and moving equipment.
