Responsibilities:

  • Conduct comprehensive assessments of patients' health status and develop individualized care plans.
  • Administer medications, perform treatments, and provide wound care as prescribed by physicians.
  • Educate patients and their families on disease management, treatment plans, and self-care techniques.
  • Monitor patients' progress and adjust care plans as needed to promote optimal health outcomes.
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and holistic care.
  • Document patient care accurately and maintain confidentiality of patient information.
  • Provide emotional support and counseling to patients and their families to promote well-being and coping strategies.
  • Participate in interdisciplinary team meetings and contribute to the development of quality improvement initiatives.

Qualifications:

  • Current Registered Nurse (RN) license in the state of CA.
  • 1-2 years of clinical nursing experience, preferably in Home Health or Hospice; or a related setting.
  • BLS/CPR certification required; additional certifications such as ACLS or wound care certification preferred.
  • Valid CA Driver’s license

Documentary Requirements:

  • Compliance with federal I-9 requirements (please bring suitable documentation with you on your first day of work verifying your identity and legal authorization to work in the United States); and
  • Mandatory Physical (within the last 6 months prior to the orientation date). You may bring a note from your physician prior to the Orientation Date
  • Mandatory TB (within the last year) OR Chest X-ray (within the last 5 years)
  • MMR immunization or titer showing immunity prior to your Orientation Date
  • Varicella immunization or titer showing immunity prior to your Orientation Date
  • Hep B immunization or titer showing immunity prior to your Orientation Date
  • TDAP vaccination or declination prior to your Orientation Date
  • Yearly Influenza vaccination or declination prior to your Orientation Date
