Case Manager
$21.12/hr
Program: Circle Lodge
Location: Montgomery County
Status: Full-Time - 40 hours
ABOUT THE ROLE
We are seeking a compassionate and driven Case Manager to join our residential behavioral health team. In this role, you will assess participant needs, coordinate services, and support individuals in achieving their recovery and life goals.
The Case Manager plays a key role in implementing treatment plans, advocating for participants, and connecting them with community resources. You will work collaboratively with a multidisciplinary team to ensure person-centered, high-quality care that promotes independence and overall well-being.
RESPONSIBILITIES
- Service Coordination & Client Support
- Assess participant needs and identify appropriate services and supports
- Develop, implement, and monitor individualized recovery and treatment plans
- Connect participants to community resources including healthcare, housing, and social services
- Support participants in achieving goals related to independence and community integration
- Transport participants to appointments as needed
- Collaboration & Teamwork
- Participate in multidisciplinary team planning and service coordination
- Collaborate with clinical, medical, and external providers
- Build and maintain relationships with community agencies
- Advocacy & Program Support
- Advocate for participants to ensure access to needed services
- Identify service gaps and recommend improvements
- Documentation & Compliance
- Complete assessments, service documentation, and reports in a timely manner
- Maintain compliance with organizational policies, HIPAA, and regulatory standards
- Attend required trainings, meetings, and supervision
REQUIREMENTS
- Minimum 2 years of experience supporting individuals in behavioral health, IDD, addiction recovery, or similar populations
- Basic to intermediate computer skills, including documentation and data entry
- Ability to work in the community and transport participants to appointments
- Strong communication and interpersonal skills
- Ability to maintain confidentiality and adhere to ethical standards
ADDITIONAL QUALIFICATIONS
- Bachelor’s degree in Human Services or a related field (or equivalent experience)
- Previous case management experience
- Knowledge of community resources and service systems
- Strong organizational and problem-solving skills
- Crisis intervention and conflict resolution experience
- Ability to work collaboratively in a team environment
- Flexibility to work evenings, weekends, or adjusted schedules as needed
