Job Details
• Night Shift 7p-7:30a, Every other weekend and holiday rotation.
• Floating Requirement – All Inpatient areas at UH Tripoint Medical Center and UH Lake West Medical Center including ICU, Stepdown and Med/Surg.
• Nurse to patient ratio 1:4-6, 1:2-3 ICU.

Job Requirements
• American Heart Association BLS required for clinical positions.
• American Heart Association ACLS required.
• Primary Source Verification of professional license and/or certification required within 30 days of start; must have valid permanent RN license.
• Proof of vaccine series or completion and approval of client medical or religious exemption form preferred but not required.
• Positive MMR titers within last 15 years required; booster protocol applies if low titer.
• Fit Test completed within 1 year of start required.
• Hepatitis B vaccination series or signed declination required.
• If positive, clear chest x-ray and current TB questionnaire required; chest x-rays valid for 5 years.
• Positive Varicella titers within last 15 years required; booster protocol applies if low titer.
• T-Dap vaccination within last 10 years required; no declinations accepted.
• 2 Step PPD OR IGRA (Quantiferon/Tspot) within last 6 months required; annual requirement.
• Proof of Flu Vaccine required from October-March; declination requires mask and specific forms.
• Non-expired photo ID required.
• UHS I-9 Attestation required.
• UHS Ohio Fingerprints required.
• UHS Education Verification required.
• UHS Employment Verification required.
• UHS Competency Exam with at least 80% score required.
• UHS Core Competencies completed within 1 year prior to start required.
• Physical/Health Statement completed within 1 year of start required.
• UHS Criminal Background Check Results required.
• UHS OIG Search required.
• UHS SAM/EPLS/GSA/OFAC required.
• UHS Drug Screen within 60 days of start required.
• UHS Module Completion Certificate required.

• Nurse to patient ratio 1:4-6 on general units, 1:2-3 in ICU.

Additional Details
• American Heart Association certifications only for BLS and ACLS.
• Temporary licenses are not accepted; must have valid permanent license.
• Attestations not allowed for license verification and education verification.
• Agency declination forms for influenza vaccine are not accepted; specific UH forms required.
• Attestations allowed for pending fingerprints with specific documentation and timelines.
• Competency exams and core competencies only need to be done once upon initial hire, not annually.
• Physical exam must state traveler is able to work without restrictions.
• Background check includes social security trace and county searches for last 7 years.
• Non-employee required safety and compliance training modules must be completed prior to clearance and renewed annually.
