Primary Responsibilities:

A Designated Coordinator (DC) at CHOICE coaches and supports people with intellectual disabilities at our centers and in the community, by providing the following services:


-Case management: Manage a caseload of approximately 10 clients from the Minnetonka-STARS location. Coordinate services by developing support plans, assessments and reports, and presenting at Annual Support Team Meetings. Be an advocate for those on your caseload, and the first point of contact for their guardians and other service providers. *This responsibility is considered a developmental component of the role and will be introduced when a caseload becomes available.


-Education and Enrichment: Plan and implement educational and life-enrichment activities and programs in CHOICE facilities. Employees are strongly encouraged to share their individual talents, interests, and hobbies with our participants. Some topics include health and fitness, cooking, sensory integration, travel, the arts, independent living skills, and much more.


-Employment Support: Provide on-site training, coaching, and workplace modifications to small crews of CHOICE participants during community-based work shifts. Crews may work in competitively paid positions as well as volunteer roles.


-Community Integration: Provide the necessary direct support, supervision, and physical assistance for participants to safely access and participate in community-based recreational and social activities. Examples include visits to museums and zoos, attending sporting events, exploring parks and trails, and participating in adaptive recreation.


-Transportation: Using a company vehicle, drive a daily van route transporting 56 participants between their homes and CHOICE in the morning (8:009:00 a.m.) and afternoon (3:004:00 p.m.). The transportation zone includes the communities of Waconia and Victoria. *This responsibility is a required component of the position and includes additional financial incentives beyond base pay.


-Care and Supervision: Provide basic health, medical, and personal care in accordance with individual support plans and protocols. Responsibilities may include toileting support and peri-care for all genders, feeding assistance, medication administration, ambulation support, and use of adaptive equipment.


Qualifications:

  • Bachelors degree (BA/BS) strongly preferred, preferably in a Human Servicesrelated field.A 2-year Associates degree plus at least 2 years of related professional experience may be considered in place of a 4-year degree.
  • At least 1 year of experience providing direct care to people with disabilities is required.
  • Valid drivers license and a clean driving record required.
  • Must pass criminal background study.
  • Ability to physically bend, stoop, reach, and lift at least 40 pounds
  • Ability to communicate and present oneself with a professional demeanor

Schedule & Hours:This position if full time, requiring a commitment to 40 hours per week, Monday through Friday, from 8:00 AM - 4:00 PM. All work is done in the field and on company premises. Remote work is not currently available.
