Job Title: Paraprofessional - Community Support Team


Location: Arbor Care Solutions, North Carolina


Position Type: Full-Time, Community-Based


Work Hours: Varying hours, Monday through Friday, with some weekend work and on-call responsibilities.


Reports To: Community Support Team Lead


Job Summary:


The Paraprofessional on the Community Support Team at Arbor Care Solutions provides essential support and services to individuals with mental health and substance use disorders. Working under the supervision of the Community Support Team Lead, the Paraprofessional assists in implementing treatment plans, providing skill-building activities, and helping clients access community resources. This role is critical in promoting client recovery and enhancing their ability to live independently in the community.


Key Responsibilities:


1.Client Support:

  • Assist clients in implementing treatment and recovery plans, focusing on developing life skills and coping strategies.
  • Provide supportive counseling and encouragement to help clients achieve their goals.


2.Skill Development:

  • Conduct skill-building activities with clients, such as budgeting, meal preparation, personal care, and social interaction.
  • Help clients develop strategies to manage symptoms and reduce the risk of relapse.


3.Resource Coordination:

  • Assist clients in accessing community resources, including housing, employment, and educational opportunities.
  • Provide transportation to appointments and activities as needed.


4.Collaboration with Team:

  • Work collaboratively with the Community Support Team to ensure coordinated care and support for clients.
  • Attend team meetings and contribute to treatment planning and progress reviews.


5.Crisis Support:

  • Provide support during crisis situations, helping to stabilize clients and connect them with necessary services.


6.Documentation:

  • Maintain accurate and timely records of client interactions and progress in accordance with organizational standards.
  • Document observations and report any concerns to the Community Support Team Lead.


7.Professional Development:

  • Engage in ongoing training and development to enhance skills and knowledge in mental health and substance use support.
  • Participate in supervision and team development activities.


Qualifications:


  • High school diploma or equivalent; additional education in a related field is preferred.
  • Experience working with individuals with mental health or substance use disorders is preferred.
  • Knowledge of community resources and services available to clients.
  • Valid drivers license and reliable transportation.

Working Conditions:
