As the In-Home Care Services Coordinator at Meals on Wheels Western Michigan, you will play a meaningful role in ensuring that older adults maintain the programmatic services to remain healthy and independent. Reporting to the Home Delivered Meals Program Manager, you will be responsible for the in‑home and phone assessments of home‑delivered meal clients.
In this role you will:
- First and foremost, align with our mission and values as an organization
- Conduct in-home and phone client assessments to determine program eligibility, food insecurity risks, and appropriate service levels.
- Maintain electronic client records, including information gathered from client assessments
- Assist clients with program navigation, and make appropriate referrals to other community resources
- Report suspected or confirmed abuse, neglect, or exploitation
- Work primarily remotely with some in-office meetings and trainings.
What you’ll Bring:
- Valid driver’s license and reliable transportation required, with the ability to travel throughout the designated service area daily
- Driving record acceptable to Meals on Wheels insurance company
- Strong time‑management skills with the ability to independently manage competing priorities effectively
- Access to a private home office space with reliable internet connection suitable for confidential work
- Exceptional communication skills, both written and verbal, with the ability to engage effectively with clients, team members, and community partners.
- Acceptable criminal background
