<li>Implement standards of care for medical social work services&nbsp;</li>
<li>Participate in patients’ plans of care.</li>
<li>Perform patient evaluations and help develop a treatment plan with patients’ physicians</li>
<li>Assess the psychosocial status of the patients as related to their illness&nbsp;</li>
<li>Make follow-up visits to assess and continue the plans of care</li>
<li>Plan interventions based on patient's needs and findings&nbsp;</li>
<li>Maintain accurate and up-to-date records</li>
</ul>
Qualifications 
<ul>
<li>Minimum one year of experience in health care and social work &nbsp;</li>
<li>Current CPR certification&nbsp;</li>
<li>Valid driver’s license&nbsp;</li>
<li>Psych experience is a plus&nbsp;</li>
