A healthcare role is available for a Registered Nurse skilled in community and small facility settings, offering an opportunity to provide comprehensive nursing care focused on health maintenance, disease prevention, and case management. This position involves assessment, planning, implementation, and evaluation of nursing support for individuals with varying health needs, as well as collaboration with multidisciplinary teams to ensure effective service delivery.
Key Responsibilities:
- Observe, assess, and develop individualized care plans for well, chronic, or acutely ill individuals.
- Collaborate with health professionals to plan and evaluate programs addressing client health needs.
- Coordinate communications among care disciplines to support health and safety.
- Manage medication processes, including ordering, reviewing, monitoring, and safe disposal in compliance with regulatory policies.
- Document patient health status and progress regularly, ensuring physician notifications when conditions change.
- Transcribe physician orders and maintain thorough nursing documentation.
- Maintain infection control standards through proper equipment and supply management.
- Supervise Licensed Practical/Vocational Nurses and ensure timely scheduling of health-related appointments.
- Maintain communication with inpatient nursing and discharge coordination during hospital stays.
- Provide training and certification updates to employees on health and safety topics.
- Ensure unlicensed staff receive person-specific training related to health maintenance and safety.
Desired Experience and Qualifications:
- Current Registered Nurse (RN) license.
- Proficient in OASIS assessment and wound care management.
- Experience managing negative pressure devices, PICC/central lines, Foley catheters, and ostomy/pleurX drain care.
- Skilled in tracheostomy care and LVAD management.
- Ability to respond on-call one weekend per month and occasional weekday after-hours calls.

This role is offered on a contract basis and includes the opportunity to impact patient care directly while working a Monday to Friday schedule from 8 a.m. to 5 p.m. Mileage reimbursement is provided for travel within Jackson and Barrow Counties.
