Position Summary
Berkley Home Health is seeking a compassionate, motivated, and patient-centered Physical Therapist to join our growing team. In this role, you will provide skilled therapy services to patients in their homes, helping them improve mobility, restore function, reduce pain, and maintain independence.
The ideal candidate enjoys building relationships with patients, working autonomously while collaborating with an interdisciplinary team, and delivering high-quality care in a home-based setting.
Responsibilities
- Perform comprehensive physical therapy evaluations and assessments
- Develop individualized treatment plans based on patient goals and clinical needs
- Provide skilled therapeutic interventions to improve strength, balance, mobility, endurance, and functional independence
- Assess home environments and recommend safety modifications when appropriate
- Educate patients and caregivers regarding treatment plans, exercises, and fall prevention strategies
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals
- Monitor patient progress and adjust treatment plans accordingly
- Complete accurate and timely documentation in the electronic medical record
- Participate in care coordination and discharge planning activities
Qualifications
- Graduate of an accredited Physical Therapy program
- Current Colorado Physical Therapist license in good standing
- Current CPR/BLS certification
- Home Health experience preferred, but not required
- Strong assessment, clinical reasoning, and communication skills
- Ability to work independently while maintaining excellent collaboration with the care team
- Reliable transportation and valid driver's license
