- Care coordination
- Disease and medication management

Work Schedule
This is a full-time position with primary hours from Monday to Friday, 8:00 AM - 5:00 PM. Weekend and holiday rotations will depend on staffing levels, with approximately two holidays per year and one weekend every 4-6 weeks.

Your Territory
As an LPN, you’ll work in the field, helping patients in their home settings. Our office is located in Roseville, MN, and your primary territory will cover the Southwest Metro area (south of Highway 394 and west of 35W).
What We’re Looking For
To thrive in this role, you’ll need:
- A current, active LPN license in the State of MN
- A minimum of 2 years of experience as an LPN
- The ability to travel within the geographic area serviced by our office, including a valid driver’s license, reliable transportation, and proof of auto insurance
- Current CPR certification

Your Responsibilities as an LPN
In this role, you will:
- Participate in ongoing interdisciplinary assessments of patients
- Provide services as ordered by the physician, following the plan of care
- Recognize and report changes in patients’ conditions to the RN
- Counsel and educate patients and families on healthcare needs
- Complete timely and accurate clinical notes, documenting patient progress
