Amoskeag Health is seeking a dedicated Primary Care Nurse to support patient care through nurse visits, triage, and communication coordination. This role plays an essential part in patient advocacy, care coordination, and nursing support across various clinical functions. The ideal candidate is a licensed RN or LPN with strong assessment, delegation, and communication skills, and a sensitivity to the cultural and age-related needs of the population we serve.
Position Summary:
- Responsible for nurse visits, patient triage (phone/walk-in), and communication with providers
- Coordinates patient messages, lab follow-ups, refill requests, dosages, and care instructions
- Supports continuity of care, urgent appointment scheduling, and follow-ups
- Collaborates closely with providers to manage team issues such as scheduling, refills, and forms
- Participates in policy development, interdepartmental meetings, safety trainings, and ongoing professional development
- Maintains accurate documentation and supports clinical operations including emergency care assistance, patient teaching, and basic nursing procedures
- Must be able to tolerate latex exposure (from vaccine vials) and perfumed products used within the environment
- Reports to the RN Manager
Qualifications:
- RN or LPN with 3 years experience
- Current NH RN or LPN license
- Community health experience preferred
- BLS Certification required
- Ability to delegate tasks to other team members
- Health assessment skills, coordination, and delegation skills necessary
