Join KeystoneCare as an Intake Coordinator, where you will play a pivotal role in ensuring seamless patient care. As the first point of contact for patients and referral sources, you will be responsible for educating consumers about our services, coordinating referrals, and facilitating effective communication between clinical staff and the community. This position allows you to make a meaningful impact on patients' lives by ensuring they receive the care they need in a timely manner.

Key Responsibilities:

  • Receive and process referrals through inbound phone calls and referral networks.
  • Inform patients and referral sources about the agency's services and capabilities.
  • Assess and ensure the agency's ability to accept referrals based on various factors such as location, clinical requirements, and insurance.
  • Maintain accurate records in the Electronic Medical Record system.
  • Assign cases to clinical teams while considering patient needs and staff availability.
  • Ensure patient care begins within mandated timelines.
  • Communicate with patients, caregivers, and referral sources regarding care initiation and modifications.
  • Adjust clinical staff schedules due to planned and unplanned absences.
  • Deliver a top-notch customer experience to both internal and external clients.
  • Follow guidance from the CEO and VP of Clinical Services.
  • Maintain confidentiality of patient information at all times.
  • Perform other related duties as assigned.

Requirements

  • Minimum of 3 years' experience in a healthcare setting, preferably in acute care, home health, or hospice.
  • Ability to multitask in a dynamic, fast-paced environment with strong communication skills.
  • Proficient customer service skills with a focus on responding to urgent healthcare needs.
  • Strong data entry and scheduling capabilities.
  • Self-motivated individual able to work independently and as part of a collaborative healthcare team.
  • Experience with electronic documentation systems and technology.
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
  • Proof of COVID-19 vaccination or an approved exemption.
  • Proof of influenza vaccination or an approved exemption.
