Location: Los Angeles, CA and surrounding areas

Employment Type: Independent Contractor / Per Visit

Compensation:

  • In-Person Visits: $250 per visit (includes 30 min visit + 30 min documentation)
  • Telehealth Visits: $140 (includes 30 min virtual visit + 30 min documentation)
  • In-Person IDG Meetings: $125/hour
  • Virtual (FaceTime) IDG Meetings: $90/hour
  • Mileage: Reimbursed beyond 20 miles from provider’s home base

As a leading provider of hospice services in {Location}, our interdisciplinary team works collaboratively to ensure comfort, dignity, and support for both patients and their families. We are currently seeking a Hospice and Palliative Home Visiting Physician to join our team and provide medical oversight and direct care for patients in their homes, assisted living facilities, and skilled nursing facilities. Compassionate and experienced Palliative/Hospice Visiting Physician to provide in-home and telehealth medical care to patients with serious or terminal illnesses. This is a flexible, non-salaried, pay-per-visit position ideal for physicians who value autonomy and patient-centered care.

Key Responsibilities:

  • Conduct in-home or facility-based visits to hospice patients, assessing and managing symptoms in collaboration with the interdisciplinary care team. Under the supervision of the Chief Medical Officer (CMO) and the Associate Medical Director (AMD)
  • Develop, review, and update individualized care plans in coordination with hospice nurses, social workers, chaplains, and other team members.
  • Serve as the attending physician (as needed) or collaborate with the patient’s primary physician.
  • Provide medical direction and guidance to hospice staff regarding pain and symptom management.
  • Complete documentation and certification of terminal illness in accordance with Medicare regulations and hospice policies.
  • Participate in IDG meetings and support the interdisciplinary approach to hospice care.
  • Perform comprehensive assessments and develop individualized care plans in collaboration with nurses, social workers, chaplains, and caregivers.
  • Provide expert management of pain and other distressing symptoms.
  • Serve as attending physician when appropriate and coordinate with referring/primary care providers.
  • Certify and recertify terminal illness for hospice eligibility, ensuring timely and compliant documentation.
  • Participate in interdisciplinary group (IDG) meetings and contribute to holistic care planning.
  • Support goals-of-care conversations and advance care planning.

Qualifications:

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) with a valid state medical license
  • Board certification in Internal Medicine, Family Medicine, or a related field (Hospice & Palliative Medicine certification preferred)
  • Experience in hospice, palliative care, geriatrics, or related settings
  • Compassionate bedside manner and excellent communication skills
  • Ability to travel locally to visit patients in their homes or care facilities
  • DEA license and current CPR certification
  • Prior experience in hospice, palliative care, geriatrics, or home-based care
  • Compassionate demeanor, strong communication skills, and commitment to team-based care
  • Reliable transportation and ability to travel locally to patient sites
