Position Overview
The Certified Social Worker provides comprehensive psychosocial assessment, care coordination, and discharge planning services for an inpatient population. This role supports patients and families by addressing social, emotional, and resource-related needs while collaborating closely with interdisciplinary care teams.
Scheduling & Work Requirements

Monday through Friday, 8:00 AM – 4:30 PM

No weekend requirement

No on-call requirement

Floating required based on operational needs

First-time travelers accepted

Local candidates accepted within a 50-mile radius
Caseload

Patient Ratio: Approximately 1:18
Required Qualifications

Minimum Bachelor of Social Work (BSW) required

Licensure not required at this time
-
Prior acute care or hospital-based social work experience preferred
Required Skills & Experience

Inpatient or acute care social work experience

Psychosocial assessments and discharge planning

Care coordination and community resource referrals

Collaboration with physicians, nursing staff, and interdisciplinary teams

Strong communication and documentation skills

Epic charting experience required
