Job Title: Physical Therapist – Rehabilitation (Travel Contract)

Location: Hyannis, MA 02601
Assignment Type: 13-Week Travel Contract
Schedule: Day Shift | Monday–Friday | 5x8 Hours (40 hours/week)
Weekend Requirement: 2 weekends OR 4 weekend days within contract duration
Holidays: Occasional holiday coverage required
Facility Type: Outpatient and Home Health Rehabilitation Services

Position Overview

"Navitas Healthcare, LLC" is seeking an experienced and compassionate Physical Therapist for a 13-week travel assignment in a rehabilitation setting in Hyannis, Massachusetts.

The Physical Therapist will be responsible for evaluating patients, developing individualized treatment plans, and delivering medically prescribed rehabilitation services to adult and pediatric populations. The role focuses on improving mobility, restoring function, preventing disability, and enhancing patient independence through evidence-based physical therapy interventions.

This position requires strong clinical judgment, excellent communication skills, and the ability to collaborate effectively with physicians, nurses, and interdisciplinary rehabilitation teams in both outpatient and home health environments.

Key Responsibilities

  • Evaluate new patients and establish measurable, functional treatment goals
  • Perform comprehensive assessments of muscle, nerve, joint, and functional abilities
  • Develop and implement individualized physical therapy treatment plans
  • Administer therapeutic exercises, modalities, and rehabilitation interventions
  • Reassess patient progress and adjust treatment plans as needed
  • Maintain accurate, timely, and compliant clinical documentation
  • Educate patients and families on home exercise programs and continuation of care
  • Collaborate with physicians and healthcare teams for coordinated patient management
  • Supervise Physical Therapist Assistants, aides, and students as appropriate
  • Ensure compliance with safety standards, infection control, and departmental policies
  • Participate in staff meetings, training sessions, and quality improvement initiatives
  • Maintain daily productivity and statistical reporting requirements
  • Support onboarding and mentoring of new rehabilitation staff when needed
  • Contribute to a culture of service excellence, accountability, and patient-centered care
  • Perform other related duties as assigned

Required Qualifications

  • Bachelor’s Degree in Physical Therapy from an accredited program
  • Active Physical Therapy license in Massachusetts
  • Current Basic Life Support (BLS) certification
  • Strong clinical evaluation, treatment planning, and documentation skills
  • Ability to work independently and within interdisciplinary teams
  • Excellent communication and patient education abilities
  • Proficiency in English (reading, writing, and speaking)

Preferred Qualifications

  • Clinical specialty certification (if applicable)
  • Experience in outpatient and/or home health rehabilitation settings
  • Prior experience in a travel or contract-based clinical role
  • Ability to support clinical leadership and staff development initiatives
