- Strong independent work skills and reliability\n
- Excellent communication and interpersonal abilities\n
- BLS certification (American Heart Association)\n

- Deliver compassionate hospice care and clinical support to patients in their homes, focusing on comfort and quality of life\n
- Conduct scheduled daily visits and respond to urgent patient needs throughout the day\n
- Assess patient needs, develop and implement individualized care plans\n
- Collaborate with interdisciplinary healthcare teams to ensure comprehensive support for patients and families\n
- Educate patients and families on care processes, symptom management, and available resources\n
- Accurately document all nursing care activities and patient interactions\n
