Job DescriptionWe are looking for an Associate Patient Care Coordinator to join our team in San Antonio, Texas. This contract position focuses on supporting patient registration and appointment scheduling processes, including telephonic outreach, insurance verification, and follow-up coordination to ensure optimal care delivery. In this role, you will play a critical part in enhancing the patient experience and meeting organizational goals.
Responsibilities:
• Conduct telephonic outreach to patients to schedule annual health screenings and other appointments.
• Make timely reminder calls for upcoming appointments, providing clear information about event locations and facility landmarks.
• Reschedule appointments for patients who cancel, reschedule, or fail to attend their scheduled visits.
• Record call dispositions accurately in the designated system, ensuring all outreach activities are documented.
• Monitor patient contact frequency to avoid excessive outreach, adhering to established guidelines.
• Utilize telephonic software for outbound calls and assist with inbound patient inquiries.
• Follow approved call scripts to ensure consistent communication during outreach.
• Collaborate with assigned providers to schedule follow-up appointments and address patient concerns.
• Direct patient messages and requests to appropriate staff, ensuring timely responses within 48 hours.
• Meet daily productivity targets while fostering positive relationships with patients, colleagues, and external partners.• High school diploma or equivalent is required.
• Familiarity with general office procedures and basic knowledge of Microsoft applications.
• One to two years of experience in data entry or a related field is preferred.
• Prior experience with medical terminology, ICD-10 coding, and electronic medical records (EMR) is advantageous.
• Strong communication and customer service skills are essential.
• Ability to manage multiple tasks efficiently and meet productivity goals.
• Comfortable sitting or standing for extended periods and lifting up to 50 pounds as needed.
• Demonstrated ability to work effectively in a detail-oriented and collaborative environment.
