LPN – Home Health
Owensboro, KY (Daviess, McLean, Ohio, Webster, Henderson Counties)
Hiring ASAP
Schedule:
- Monday – Friday, 8:00 AM – 5:00 PM
- No weekends | No on-call
Comp & Details:
- Productivity: 32 points/week
- Mileage: $0.43/mile
Key Responsibilities:
- Provide skilled nursing care in the home setting per plan of care
- Monitor vital signs and report abnormal findings
- Administer medications, feedings, oxygen, and perform ostomy care
- Assist with care plans established by PT, OT, and Speech Therapy
- Ensure infection control practices are followed at all times
- Document all care, patient progress, and outcomes accurately
- Coordinate supplies, medications, and equipment needs
- Monitor and report incidents; complete documentation as required
- Ensure all patient needs are met in accordance with physician orders
Requirements:
- Active Kentucky LPN license
- Valid driver’s license & reliable transportation
- 1+ year home health experience preferred
- Experience with HCHB strongly preferred
- Wound care experience a plus
- Strong clinical and documentation skills
