Coverage Area: Greater Grand Rapids + Kalamazoo as needed

As a Hospice Admissions Nurse, you play a crucial role in providing compassionate care to patients and families during a sensitive time. Your primary responsibility is to facilitate the admission process for individuals entering hospice care, ensuring that they receive the support and services they need by assessing patients medical needs and developing a plan of care to maximize the comfort of the patient. Additionally, you will be a designated source for addressing concerns and educating patients and their loved ones about hospice philosophy and available resources.
Required Experience for an Admissions RN:
- Current RN license in state of practice
- Minimum 2 years of experience of clinical nursing experience, in hospice or palliative care.
- Excellent communication and interpersonal skills
- Compassionate, empathetic, and patient-centered approach to care
- Working knowledge of computer software programs.
- Strong assessment and critical thinking skills, with the ability to make sound clinical judgements
- Ability to sit for extended periods
Primary Responsibilities of an Admissions RN:
- Conduct comprehensive assessments of patients referred for hospice services to determine their eligibility and appropriate care needs.
- Coordinate admission to hospice while assisting with implementation and coordination services.
- Provide education and counseling to patients and families about hospice services, end-of-life care options and advance care planning to assist in informed decision-making and an understanding of available resources.
- Obtains relevant information and supporting documentation to include medical records, vital signs, and patients’ functional status to support eligibility.
- Assess and manage patient’s symptoms, and provide interventions, collaborating with healthcare professionals if needed.
- Maintain accurate and timely documentation of assessments, care plans, interventions and communications with patients’ families and healthcare providers in accordance with regulatory requirements and organizational policies.
- Collaborate effectively with interdisciplinary teams, including physicians, nurses, social workers, chaplains, and other healthcare professionals to ensure coordinate plan of care.
- Offer emotional support and guidance to patients and families throughout the hospice journey, addressing concerns, providing reassurance, and facilitating access to additional support services as needed.
- Provide immediate support and interventions during times of crisis or acute symptom exacerbation, ensuring patient and family needs are addressed promptly and effectively.
- Protect and honor patient confidentiality.
- Participate in training, business meetings and mentoring new staff.
- Complete all assigned continuing education by assigned completion date.
- Other job duties as assigned.
General Working Conditions:
This position entails sitting for long periods of time. While performing the duties of this job, the employee is required to communicate effectively with others, sit, stand, walk and use hands to handle keyboard, telephone, paper, files, and other equipment and objects. The employee is occasionally required to reach with hands and arms. This position requires the ability to review detailed documents and read computer screens. The employee will occasionally lift and/or move up to 25 pounds. The work environment requires appropriate interaction with others. The noise level in the work environment is moderate. Occasional travel to different locations may be required.
